SBD Foundation Guidelines

To apply for a grant, an eligible organization must submit a completed application. The Grants Committee will meet once a year to review requests.

All applicants must provide the following:

1. Letter from agency requesting the grant - stating need for which the grant will be used
   
2. Program Budget
   
3. Timeframe for which funds are needed
   
4. Documentation of additional funding sources for program (if applicable)
   
5. Most recent financial statement and most recent IRS Form 990
   
6. List of Board of Directors and Officers and addresses
